We initiate our move in process that includes, but is not limited to, filling out a life story, asking detailed questions of loved ones if the client themselves cannot answer, doing a thorough assessment of the potential clients which includes levels of personal care, what they like, dislike, (past and current) and what their current abilities are. Also included in this move in process is an individualized activity plan for the resident.

After gathering all pertinent information the Administrator can then successfully develop person directed care plans for the potential client. Special Memory Care Activity Calendars will be made around specific activities that the residents enjoy, which will allow us to offer things that are person directed for a better quality of enjoyment.

After the initial assessment is completed, we update at 30 days, again at 60 days, and then every 90 days there after. The only other situation that would require an immediate care plan adjustment would be a significant change of condition.

We at Elderberry Square, feel that family is the root of all success and so we offer support groups at least one time per month for families with loved ones in our Memory Care units. This allows family members to lean on one another and share stories and offer support. Elderberry also makes special occasions open to all family members with cordial invites to join us.
